I live in a rural area comprised of critical access hospitals in a 60 mile radius. The one I work at is one of the biggest in the area and one of the few to also offer OB. It's my first RN job and I've been there for almost 9 months. The job I was hired for was a hospital RN. We float med/surg, ER, "ICU", and OB. In general they allow people to spend the majority of hours in their area of interest. I was very clear in my interview that my interests are mainly ER followed by ICU. I have a 5 year background in EMS which helped to give me a leg up in those areas. I was able to get to ED quite quickly from med/surg and management was impressed with my knowledge and skillset. It's a small enough ER that they only have 1 nurse staffed each shift for 5 rooms. Somedays I feel like I'm nearing the limit on what I can learn from others there and have really had to teach myself any new skills. Unfortunately the past couple months I've been floated back to med/surg more often to train new staff as they have a high turnover rate and my manager says I'm one of their strongest preceptors.

I loved being in the hospital and working with patients. However pretty quickly I found myself kind of disappointed with the level of depth at which we were learning things. I felt like I was learning the "cliff notes" of medicine, and did not really understand the why behind a lot of the diseases/treatments. I really want to understand things on the cellular level and learn the science behind everything. If some process should be learned like A --> B --> C --> D --> E --> F --> G, as a nursing student I felt like I learned it A --> D --> G. And that really bothered me. I just am someone that wants to learn as much as I can about something. As these concerns festered, I also had an experience during my Women's Health rotation. I got to witness an emergency C-section. The OB/GYN physician was incredible and as corny as it sounds I was just so inspired by her. I was super excited by what she did and ended up shadowing her. I loved that she was an absolute EXPERT on the female reproductive organs/related issues.

What makes it difficult for me when applying to any job, I get so nervous. I've never been interviewed in my life. The jobs I've worked for in the past (non medical) they just hired me on the spot. So idk what they ask during medical interviews, also I heard sometimes there's more than one interviewer and that makes me so damn nervous just thinking about it 😩 I'm a very shy and quiet person that is hard to make friends because I'm shy. I'm ready laid back and a cool person once I get comfortable. Any hospitals with good benefits that are night shift hiring? I thought I'd ask here and see if anyone working somewhere knows if their unit is hiring. If you work night shift, what hospital and unit are you on? Do you like it? What is the nurse to pt ratio like? What is the pt acuity like? Do you enjoy your job? How are your coworkers? (I'm hoping to find a job where my coworkers are nice and understanding because I ask a lot of questions to be safe).

PGY-2 - Similar to PGY-1 will be a mix of in the ED and off-service rotations. You're now more senior, so most programs by this time will have you running medical resuscitation activations, and in my program, this is when we start running traumas as well. You'll make a trip to additional ICUs or you'll return to the ones you had been in previously as a senior resident. My program had a PICU month which I found incredibly valuable that not every program has. We also had an elective this year which made a huge difference for my future career - I'll be pursuing a fellowship that I likely would not have been able to do if I hadn't rotated second year. Other than that, the focus this year is on developing confidence and flow. EM sees a lot of sick patients quickly, and you need to be able to cut through a lot of extraneous information to get an appropriate workup and disposition nailed down quickly. PGY-3 - You're the boss now. You spend most of your year in the ED with a bit of time to wrap up residency related tasks such as research and possibly another advanced elective.

We were really scraping by under the assumption... my husband even said this over and over... he needed to make more money and get a different job. He's been saying this for a year now. Things started going wrong pretty quickly after my first trimester. Right around 20 weeks I needed to go from a full time benefited employee to an on call position. Two nurse appointments a week, two/three OB appointments every month, extra ultra sound appointments at a perinatalogist just drained my PTO and someone needs to be there in my office. No one else can just do my job. They needed a full time employee. My income went down to less than 1/2 of what I was making quickly. As of October 1st I will be on early maternity leave, due to my OB ordering me to rest so I don't go into early labor. So there will be no income after that. Bills got out of control. We maxed our credit cards on daycare bills (keep in mind I'm still working 3 days a week) After October 1st, the kids won't be attending. But we still owe the daycare money.
